Permission Forms for Stepfamilies

 

 

Because biological parents and stepparents don't have the same legal ties to children, special permissions are necessary for stepparents to, for example, give medical consent for a child to be examined by a doctor, take children on overnight trips away from home, and pick up school records.

  1. Stepparent Medical Permission - gives permission to stepparents to make medical decisions when necessary for a stepchild.

  2. Grandparent Medical Permission - gives permission for a grandparent to make medical decisions when necessary for a stepchild.

  3. Caretaker Medical Permission - gives permission for a responsible adult (e.g., stepparent's father or mother) to make medical decisions when necessary for a stepchild.

  4. Travel Permission - gives a stepparent permission to travel with stepchildren (acknowledges the biological parent's awareness of the travel).

  5. School Duplicate Information - gives schools permission to share information about a child with both households, including parents and stepparents.

dose any of this cover juvinal system talking to step parents,and also can I write and notorise a paper for my fiancee of 7 yrs. to be able to handle situations legaly with my son , my son calls him dad and see's him as his dad !

No it does not. The only thing to my knowledge that offers "legal" rights is formal adoption. Sounds like you need to consult an attorney on this.

On the medical permission form, there is space for two parent signatures. Does this mean both biological parents have to sign this form. I have sole physical custody, but joint legal custody of my boys. My children's father is not in the picture at the time and I do not have any idea how to contact him. Is my signature enough to make this form legal for my husband to seek medical care for my children?

It is always better if both biological parents sign the form, especially if you have joint custody. Each state is different in whether both parents must sign the form. Please get an opinion from an attorney in your state.

the school one is irrelavant if the child lives in the same household as the stepparent. FERPA places a higher value on the rights of the stepparent that it does the child's real parent. ridiculous, but true.
