Church Name:  Include denomination (if any); mailing address; phone number; and general email address; web address; typical Sunday worship attendance.

Staff Contact:  Key staff contact; email address; phone number.

Volunteer Leader/Couple:  List any lay leaders; email address; phone number.

Pre-Stepfamily Counseling:  Does your church/ministry offer unique pre-stepfamily counseling?  Tell us about it.

Stepcouple Marriage Mentoring:  Does your church/ministry have a mentoring program for couples in stepfamilies?  Tell us about it.
